## Quick answer

Start with your county, your doctors, and your drugs. Compare the same facts on every plan: network, drug list, total yearly cost, and the CMS star rating. No single plan fits everyone in North Carolina.

## Start with your county, not the state

CMS lists 179 Medicare Advantage plans in North Carolina for 2027 in its landscape file, up from 175 in 2026. That statewide count misleads more than it helps. Plans are approved county by county, so the list you can actually join is set by your home address. Guilford County and Wake County do not offer the same plans. Run every comparison at your ZIP code, in the Medicare plan finder at medicare.gov, before you compare anything else.

## Check your doctors and your drugs first

A plan that excludes your doctor or drops your drug is the wrong plan at any premium. Ask three questions of every plan: is each of my doctors in network, and what do I pay if one is not; is each of my drugs on the formulary, and on which tier; and is my pharmacy preferred or standard. HMO plans generally require in-network care except in emergencies. PPO plans cover some out-of-network care at higher cost. Get the answers in writing from the plan's own documents, not from an advertisement.

## Read the total year, not the premium

A $0 premium plan is not a $0 plan. You still pay the Part B premium, plus copays, coinsurance, and any drug deductible when you use care. Add 12 months of premium to your expected copays and drug costs, then compare that yearly total. CMS caps what a 2027 plan can set as its in-network out-of-pocket limit at $9,850, up from $9,250 in 2026, and at $14,800 counting in-network and out-of-network care combined for PPO plans. The median in-network limit rose to $6,600 in 2027 from $5,900 in 2026, in an analysis of the CMS landscape files. CMS projects the average Advantage premium at about $12 a month in 2027, down from $14.37 in 2026. Averages describe the market, not your plan. Your Annual Notice of Change has your plan's real numbers.

## What the star ratings do and do not tell you

CMS rates Medicare Advantage contracts from 1 to 5 stars each fall, on quality and performance measures such as preventive care, chronic condition management, and member experience. The ratings published in October 2026 apply to 2027 plans. In North Carolina, 172 of the 179 plans have a 2027 rating, 125 are rated 4 stars or more, and 2 are rated 5 stars. Ratings are per contract, so every plan under one contract shares its rating, and a new or small contract may have no rating yet. A 5-star rating does not mean your doctor is in network or your drug is covered. Use ratings as a quality check after the network and drug checks pass, not before.

## Extra benefits are real but secondary

Many Advantage plans add dental, vision, hearing, over-the-counter allowances, and fitness benefits. These extras have their own limits, networks, and rules, and they change every year. A grocery allowance never outweighs a plan that excludes your cardiologist or drops your prescription. Compare extras last, after network, drugs, and yearly cost.

## Your 2027 decision dates

Annual Enrollment runs October 15 through December 7, 2026, for coverage starting January 1, 2027. If you are already in a Medicare Advantage plan, the Medicare Advantage Open Enrollment Period runs January 1 through March 31, 2027, and gives you one change. A move, a plan leaving your county, or losing other coverage can open a Special Enrollment Period outside those windows. North Carolina enrollment help is local: Christian walks Triad residents through the comparison at no charge, and the state's free SHIIP counselors do the same in every county.

## Compare the same facts on every plan

| What to check | Why it matters |
| --- | --- |
| Doctors in network | Out-of-network care can cost more than the premium saves |
| Drugs on the formulary | A drug off the list, or on a high tier, changes your year |
| Yearly out-of-pocket limit | The ceiling on covered in-network care, at most $9,850 in 2027 |
| Star rating | A quality signal for the contract, not a guarantee of fit |
| Extra benefits | Nice to have, never the deciding factor |

## Frequently asked questions

### How many Medicare Advantage plans are in North Carolina for 2027?

CMS lists 179 plans statewide in its 2027 landscape file, up from 175 in 2026. Your county list is much smaller, because plans are approved county by county. Check your ZIP code in the Medicare plan finder.

### What is the highest out-of-pocket limit a 2027 plan can set?

CMS caps the in-network limit at $9,850 for 2027, up from $9,250 in 2026. For PPO plans, the combined in-network and out-of-network limit is capped at $14,800. Most plans set lower limits, so read your plan's number.

### Do star ratings rank plans from best to worst?

No. Star ratings measure a contract's quality and performance, from 1 to 5 stars, on measures such as preventive care and member experience. They do not measure whether your doctor is in network or your drug is covered.

### Is a $0 premium Medicare Advantage plan free?

No. You still pay the Part B premium, and you pay copays, coinsurance, and drug costs when you use care. A $0 premium plan can still cost thousands in a heavy year, up to its out-of-pocket limit.

### When can I change my Medicare Advantage plan for 2027?

During Annual Enrollment, October 15 through December 7, 2026, for coverage starting January 1, 2027. If you are already in an Advantage plan, the Medicare Advantage Open Enrollment Period, January 1 through March 31, 2027, gives you one change.
